Medical Paperwork Requirements
Prior to participating in any KHS official team related functions (including pre-season training and tryouts), the student-athlete must complete the following in order to be medically eligible:

Medical Paperwork Submission
Complete and submit current copies of the following paperwork to the Athletic Trainers ( ). You will receive a confirmation email within 48 hours. If you do not receive a confirmation email, please re-send/follow-up in case forms went to the SPAM folder. 
  • Concussion Management Study Form (1 page): One per school year; Participation in the Concussion Management Study is optional, however, the form itself must be completed. To use the fillable PDF feature of this form, download the form to your computer to complete and submit. 
  • DOE Physical Examination for Athletes Form (Physical form) (2 pages): A current Physical Examination for Athletes form must be on file.  The physical is good for one year from the date of the examination and shall cover all sports available during that time frame. 
  • Consent, Release, and Assumption of Risk and Emergency Information Form (CAR Form) (2 pages): This form is valid for one sport for a single school year. Multiple CAR forms must be submitted if interested in participating in more than one sport. This form will be on file with the coaching staff at all practices, athletic contests, and team functions in the event of an emergency. To use the fillable PDF feature of this form, download the form to your computer to complete and submit. 
Concussion Baseline Test (Baseline tests will be scheduled through the coaches once practices begin)
All student-athletes interested in participating in collision and/or contact sport are required to take a concussion baseline test prior to participation. Baseline concussion tests are valid for two years from date of test. Baseline test may also be retaken if the student-athlete sustains a concussion within the two years. All freshman, juniors, and anyone who did not take the test last year will need to take a new baseline test.

    • Cheer
    • Football
    • Judo
    • Soccer
    • Softball
    • Volleyball
    • Water Polo
    • Wrestling
